Services and Responsibilties
Services Provided and Responsibilities of the Therapist are to:
- Provide cognitive behavioural therapy (CBT) informed services to the Child and family.
- The therapy sessions will be conducted in a professional and ethical manner.
- Maintain confidentiality as outlined in this Agreement.
- Communicate regularly with the Parent(s)/Guardian(s) regarding the Child’s progress with consent of the child.
- May include individual therapy sessions, family therapy sessions, and other therapeutic activities as deemed necessary by the Therapist.
- Treatment plan that is tailored to the Child’s needs.
Responsibilities of the Parents/Guardians are to:
- Ensure the Child attends scheduled therapy sessions.
- Participate in family therapy sessions as requested by the Therapist.
- Provide accurate and relevant information about the Child’s history and current condition.
- Pay for services as outlined in the Payment Terms section.
Confidentiality
All information shared during therapy sessions will be kept confidential, except in the following situations:
- If the Therapist believes the Child is in danger of harming themselves or others.
- If there is suspected child abuse or neglect.
- If required by law or court order.
- If the Parent(s)/Guardian(s) or child consent to the sharing of relevant information between the Therapist and other healthcare providers or school personnel as needed for the Child’s treatment.
Payment Terms
The cost of therapy sessions agreed at time of booking. Payments are due at the time of each session unless otherwise agreed. Payment method will be agreed at time of booking.
Cancellation Policy
If a session needs to be cancelled, please provide at least 48 hour’s notice. Cancellations with less than 24 hour’s notice will be charged the full session fee, except in cases of emergency or at the discretion of the therapist. If a session is missed and the therapist has not been notified will be charged the full session fee.
Emergencies
The Therapist is not available for emergency or provide a crisis services. The therapist recommends contacting local emergency services like A&E.
Termination of Services
Either party may terminate this Agreement at any time by providing 14 days written notice. The Therapist may terminate services if it is determined that the Child is not benefiting from therapy, or if there are repeated cancellations or non- compliance with the terms of this Agreement.
